  1. On the identID treatment page, click the specific problem you’d like to treat.
  2. You will be directed to a questionnaire where you’ll provide a brief overview of your condition, symptoms, and medical history
  3. You will then be given purchase options which may vary depending on the type of consult you are requesting and your choice of shipping. (NOTE: no prescription is guaranteed without first being evaluated by an MD/NP licensed in your state) Depending on the condition, individuals are directed to different paths below:
    1. If you submitted a questionnaire seeking treatment for erectile dysfunction, hair loss, performance anxiety congratulations! The hard part is over! Our prescribers will evaluate the information you submitted and determine if you qualify for medical treatment. No video or phone consultation is needed. At this point you will be directed to creating your patient portal login where you can review your order history, request prescription refills or chat with a prescriber. Our goal is to have one of our doctors or nurse practitioners review everything within one business day (Monday-Friday 8am-5pm Eastern and Saturday-Sunday 8am-12pm Eastern). Prescribers may need additional information from you so be sure to check your messages until your prescription is verified.
    2. If you submitted a questionnaire for insomnia or weight loss at this point you will be directed to a booking page to schedule a video consultation with a prescriber. Video consultations are required for all conditions that may be treated with controlled substances such as Zolpidem (generic Ambien) or Eszopiclone (generic Lunesta). During this booking process you will be directed to creating your patient portal login where you can review your order history, request prescription refills or chat with a prescriber.
    3. If seeking treatment for testosterone replacement at this point one of our team members will send you a lab acquisition form to be completed at any Empire Lab location. If you opted to supply your own blood work please either upload it to your patient portal or email it to office@identidhealth.com. You will also be directed to create your patient portal login where you can review your order history, request prescription refills or chat with a prescriber. Once your blood work is received you will then be sent a schedule to book your video consultation with one of our providers. Video consultations are required for all conditions that may be treated with controlled substances such Testosterone.
  4. If medication is to be prescribed, you can then sit back and wait for the medication to be shipped directly and discreetly to your doorstep.

Currently IdentID can only treat patients in New York and New Jersey. In the near future we will be able to prescribe to Florida but we are constantly expanding to eventually cover all 50 states. Please input your email so we can contact you when we have services in your home state.

The IdentID team includes licensed physicians, NPs, PAs, and pharmacists. Your specific provider will depend on your particular concern and your state of residence. Each provider utilizes evidence-based guidelines to deliver high-quality care. They are all board certified and meet all state/national regulated credentialing.

IdentID does not accept insurance, nonetheless pricing is always transparent and affordable with lower costs compared with many of our competitors.

In some cases, insurance may be utilized for services not offered through IdentID. These patients may pay for lab work out of pocket or contact their insurance provider for cost assistance.
